Convert a weight-based dose (mg/kg) into tablets

How it works

This calculator converts a weight-based medication order (mg/kg) into a total dose in mg, then converts that dose into a tablet amount using the available tablet strength (mg/tablet).

It’s commonly used in paediatrics, antibiotics, analgesia, and any protocol where dosing depends on patient weight.

Formula

Total dose (mg) = Weight (kg) × Dose (mg/kg)
Tablets = Total dose (mg) ÷ Tablet strength (mg/tablet)

Worked examples

Example 1: 70 kg, 10 mg/kg, 500 mg/tablet

Total dose = 70 × 10 = 700 mg
Tablets = 700 ÷ 500 = 1.4
Answer: 1.4 tablets (follow policy for rounding/splitting)

Example 2: 25 kg, 15 mg/kg, 250 mg/tablet

Total dose = 25 × 15 = 375 mg
Tablets = 375 ÷ 250 = 1.5
Answer: 1.5 tablets

Example 3: 80 kg, 20 mg/kg, 400 mg/tablet

Total dose = 80 × 20 = 1600 mg
Tablets = 1600 ÷ 400 = 4
Answer: 4 tablets
