Medication and dosage calculations often produce more decimal places than you can actually use. For nursing dosage-calculation rounding, the safest general rule is: preserve verified source values and intermediate results, then round the final answer only to the precision required by the question, authorised reference, device or local policy.

Rounding too early changes the number that the next step uses and can shift the final answer.
What does rounding mean?
Rounding replaces a long decimal with a nearby practical value. The place you round to matters.

The calculator may show six or eight decimal places. That does not mean your final medication answer should contain all of them.
The basic rounding rule
Choose the place you need to keep, then look at the digit immediately to its right:
- 0 to 4: keep the rounding digit the same.
- 5 to 9: increase the rounding digit by one.
1.42 → 1.4 to the nearest tenth
1.47 → 1.5 to the nearest tenth
2.346 → 2.35 to the nearest hundredth
Rounding is not the same as truncating
To round, keep the requested place and inspect the next digit. To truncate, you simply cut digits off. Those two methods can give different answers.
0.846 mL → 0.85 mL to the nearest hundredth
0.846 mL → 0.84 mL by simply deleting the 6
Why you normally round at the end
OpenStax nursing guidance recommends avoiding rounding during intermediate steps because each early rounding decision changes the value used in the next part of the calculation.
For example, imagine an intermediate result is:
If that value still needs to be multiplied by 3:
1.666666 × 3 ≈ 5.0
1.7 × 3 = 5.1
The difference is small here, but longer calculation chains can magnify it. Every early rounding step gives the next calculation a slightly changed number.
How many decimal places should you use?
There is no single rounding rule that is correct for every medicine, patient, device and workplace. Some educational resources describe common conventions, but clinical practice must follow the precision required by the authorised order, product information, measuring device and local policy.
Depending on the task, you may be asked for:
- the nearest whole number,
- the nearest tenth,
- the nearest hundredth, or
- a device-specific increment.
Calculated precision is not the same as measurable or programmable precision
A calculator might display 1.237468 mL. That is mathematical precision. The amount that can be measured or programmed depends on the authorised syringe, cup, pump or other device and the increments it can reliably represent.
FDA medication-error guidance and oral-liquid safety work support matching dose expression to an appropriate measuring device rather than assuming that every calculator digit can simply be used as written.
Do not let a small non-zero amount disappear as zero
A generic display rule can be unsafe if it hides the fact that the mathematical result is non-zero. For example, 0.0046 mL shown to two decimal places would display as 0.00 mL.
Fraction maths is not the same as medication-direction wording
Mathematically, 1/2 = 0.5. In medication directions, however, numeric fractions and decimals are not automatically interchangeable. Current ISMP guidance identifies forms such as 1/2 tablet and 0.5 tablet as error-prone dose designations and recommends clear wording such as half tablet where that direction is appropriate.
A calculation such as 1.5 tablets still does not prove that the specific product can be split. Medication-direction wording and product splitability are separate safety checks from the arithmetic.

If the decimal point in .5 mg is overlooked, the dose can be mistaken for 5 mg. The Australian Commission on Safety and Quality in Health Care recommends using a zero before the decimal point for values less than one, and the Joint Commission uses the same safety principle.
Avoid trailing zeros in medication doses
A trailing zero can become dangerous if the decimal point is missed: 5.0 mg may be misread as 50 mg. Australian medication-terminology guidance, ISMP and the Joint Commission all identify trailing-zero dose notation as a medication-safety risk.
Assessment formatting and medication documentation are different contexts
A school or exam question may deliberately ask you to show a pure maths answer to a fixed number of decimal places. Medication documentation follows medication-safety notation instead. Do not add an unnecessary trailing zero to a medication dose merely to make it look like a two-decimal-place worksheet answer.
Rounding and safe notation are two different decisions
Imagine the exact calculated value is:
If the instruction is to round to the nearest tenth:
Choosing 0.5 mg is the rounding decision. Writing 0.5 mg instead of .5 mg is the notation-safety decision.
Worked example: rounding a final liquid volume
A calculation gives a final liquid dose of 3.276 mL. The question says to round to the nearest tenth.
- Keep the tenths digit: 3.2.
- Look one place to the right: the hundredths digit is 7.
- Because 7 is 5 or greater, round the 2 upward.
The important point is not just that 3.276 becomes 3.3. It is that the earlier dose calculation kept its precision until the final volume was known.
Worked example: do not round a useful input without a reason
Patient weight: 17.4 kg
Order: 7.5 mg/kg
Concentration: 50 mg/mL
First find the dose:
Then find the liquid volume:
If the final instruction says to use the nearest tenth:
We did not round 17.4 kg to 17 kg first. Inputs should not be rounded simply because a simpler number feels easier to calculate with.
Whole-number metric expression is a unit-conversion question, not a rounding shortcut
Some medicines-terminology guidance prefers a whole-number metric expression where an exact equivalent is appropriate. For example, changing 0.5 g to 500 mg is an exact unit conversion, not a rounding step.
Never move the decimal point or change units casually just to avoid a decimal. Preserve the exact quantity and use the authorised terminology. See the Unit Conversions Guide for the conversion method.
What about infusion pumps?
Pumps do not all accept the same increments. A calculated rate such as 23.47 mL/hr may need to be entered differently depending on the authorised device configuration, medicine and local policy.
The calculation tells you the mathematical rate. The device and clinical instructions determine what precision can actually be programmed.
Common rounding mistakes
Keep useful precision through the calculation and round at the end unless the method explicitly requires otherwise.
More digits do not automatically make the final answer better.
Use the precision required by the actual medicine, device, assessment or local policy.
Use a leading zero: 0.5 mg.
Avoid unnecessary trailing zeros in medication doses.
A decimal tablet result does not prove that the tablet can be divided that way.
The final practical dose must be compatible with the authorised measuring/programming device.
Deleting digits without checking the next digit can produce a different answer.
If the authorised method/device cannot represent the amount, verify the preparation or measurement method rather than hiding the value as zero.
Medication documentation should not gain an unnecessary trailing zero merely to display a fixed number of decimal places.
Use rounding safely

For real medication administration, follow the current prescription, product information, authorised medication reference, device capability and local policy. If a calculated answer cannot be measured or programmed as written, stop and verify the required method rather than inventing a rounding rule.
