- Recognise that a new bag or changed concentration requires a fresh calculation rather than copying the old pump-rate ratio.
- Use a simple proportion check only when the concentration and any verified dosing weight stay the same.

Why this matters
These questions become confusing when the old pump rate, new medication order, concentration and times are mixed together. Keeping BEFORE and AFTER separate makes it clear which numbers belong to which part of the calculation.
