Medication percentage calculation practice
A percentage means per 100. For a weight-in-volume concentration, 1% w/v means 1 g in 100 mL, which equals 10 mg/mL. That shortcut applies to w/v only; v/v and w/w describe different quantities.
Formula
for % w/v: concentration (mg/mL) = percentage × 10
The percentage basis must be supplied or verified before using a conversion. Percentage strength describes concentration and does not independently determine a medication dose, route or dilution plan.
Sample question
A fictional solution is labelled 2.5% w/v. What concentration does that represent in mg/mL?
2.5 g/100 mL = 2500 mg/100 mL = 25 mg/mL
Answer: 25 mg/mL

Identify it
Recognise the lesson’s key idea, the values that belong in the calculation and the unit the answer needs.
Calculate it
Use clearly stated orders and supplied information to complete straightforward calculations.
Interpret it
Handle frequency, totals, conversions, order changes and realistic but relevant detail.
Reason through it
Reason through dense ward stories, changed orders, competing numbers and multi-part calculation chains.
Learned means the lesson is complete. Practised means you have attempted questions. Secure requires a strong unassisted run. Mastered requires strong evidence again after time has passed. Review due means a previously stronger skill is ready for retrieval again.
